Health insurance

Before you complete the form and pay for the service:

1. Read about how health insurance works in the Netherlands.
2. Check what your own risk pool is – and decide what risk pool you want to have.
3. Choose your insurance with the help of a comparison engine. If you don’t care, mark it in the form.

What to prepare:

1. Personal data.
2. The Dutch account number.
3. If you are not registered in the Netherlands, a copy of your identity card, your current employment contract and the latest salary paper.

Be prepared to make the payment at the end of the form. Payments accepted are iDeal and Przelewy24 (Polish provider).
